Provides a range of services to children, young people and their families.
The centre is opening soon
Merrylands Community Health Centre has undergone extensive refurbishments. The stunning result provides the Western Sydney Local Health District (WSLHD) community even better access to a range of primary care services including child and family health, podiatry, mental health and drug health services.
Modern design, new flooring, and stylish furniture and fittings complement a refurbishment program incorporating electrical works, data, plumbing and security services.
Topping off improvements inside the Merrylands HealthOne project is the installation of a new lift. Outside, new paintwork and a refreshed façade welcomes the community.
All provide a more comfortable, professional environment befitting the high level of quality health care and services.
The centre is scheduled to reopen soon.

How to access this service
The Central Referral Service (CRS) is the central access point to Complex, Aged and Chronic Care and Child & Family Health Community Health Services and enables GPs, patients, hospitals, families and health providers to register their request for services into Community Health Services for clinical triage.

CRS receives and registers requests for referrals for WSLHD Community Health clinical services such as Nursing, Women’s Health, Speech Pathology, Occupational Therapy, Physiotherapy, Podiatry, Audiometry, Counselling, Aged Day Care and Palliative Care.
Central Referral Service phones are staffed from 8.30am to 4.30pm Monday to Friday.

Why does CRS ask so many questions?
CRS uses Community Health Service criteria to determine the eligibility of the referral request. Demographic, family/support, legal issues, risk issues, safety issues and presenting health information is required for the client registration and clinical allocation to the appropriate Community Health service. Some information requested is required by the Ministry of Health and funding agencies.
