Care Navigation

Youth Health works in Emergency Departments (ED) across Auburn, Blacktown, Mount Druitt, and Westmead Hospitals to assist with care navigation for marginalised young people to reduce re-presentations to ED. We work with young people to improve their access to primary and community health care, reducing their reliance upon ED. This role has a focus on young people without a mental health diagnosis or who do not meet mental health service criteria to enhance access to primary and Community Health services.

Youth Health also supports care navigation and coordination of services for marginalised young people to improve their access to the right healthcare, at the right place, at the right time. This can include primary health services, NDIS supports, acute specialist services, mental health, and community health services.
