Enquiries & Referrals

Who can refer to our service?

We accept self-referrals and assisted referrals through parents, family members, friends, GPs, school teachers, counsellors, or other service providers, with the young person’s consent.

How do I refer to your service?

Referrals to our service can be made:

  1. via phone
  2. in person through our Worker on Duty (WOD)
  3. by downloading and completing the Youth Health triage/referral form and emailing it to the attention of 'Worker on Duty' at either
    High Street Youth Health Service, Harris Park   WSLHD-HighStreet@health.nsw.gov.au
    or
    Western Area Adolescent Team, Mount Druitt   WSLHD-WAAT@health.nsw.gov.au

Group work referrals can be made:

  1. Via phone
