Inpatient services - rehabilitation (SMART) program
What is SMART?
SMART stands for (Specialist Management with Acute Rehabilitation Treatment).
Aims of the treatment
SMART provides patients with active rehabilitation and therapy interventions to:
- achieve a good recovery
- prevent any further physical weakness
- assist in a safe and independent return to usual accommodation.
- assist transition to further rehabilitation.
Who is SMART for?
The SMART program was established at Westmead Hospital in 2010 for inpatients who benefit from early rehabilitation after general and cardiothoracic surgeries and for patients with neurological or oncological conditions.
Currently the SMART program is offered to patients in the general surgical wards (B3a/b/c), cardiothoracic ward (D3c), neuroscience ward (D5c), oncology ward (C5c), renal/urology ward (A6a), and gastroenterology ward (D3a).
Patients are referred to SMART by their treating medical team and their appropriateness for SMART is determined by the SMART consultant and registrar.
SMART team members
Include:
- rehabilitation physician and registrar
- nurse coordinator
- physiotherapists
- occupational therapists
- social worker.
