Who we care for

We care for people who live or attend preschool/school in the Holroyd or Parramatta Local Government Area.

There are no age restrictions for those seeking counselling services.

The following services have age restrictions;

  • Child and Family Health Nursing is for babies and children aged 0-5 years
  • Speech Pathology is for babies and children aged 0-7 years
  • Chronic and Complex Podiatry is for clients aged 16 years and above

What we do

Merrylands Community Health staff work in partnership with schools, General Practitioners, hospitals, non-government agencies and other organisations to provide care at the Community Health Centre, in people’s homes or in other venues, such as Early Childhood (Baby Health) Centres.

We also specialise in health promotion, prevention and early detection to assessment, treatment and continuing care.

How to access this service

The Central Referral Service (CRS) is a single point of entry for Western Sydney Integrated & Community Health.

CRS is streamed along the lines of the clinical initiatives of Complex, Aged and Chronic Care, Community Palliative Care, Child and Family Health Community Health Services, COVID-19 Home Testing, Residential Aged Care Facilities and enables GPs, NSW Ambulance, Aged care facilities, patients, hospitals, families and health providers to register their request for services into Community Health Services for clinical triage.

CRS receives and registers requests for referrals for WSLHD Community Health clinical services such as Nursing, Speech Pathology, Occupational Therapy, Physiotherapy, Dietetics, Podiatry, Audiometry, Counselling and Palliative/Supportive Care.

CRS does not provide clinical assessment or triage services.

Central Referral Service phones are staffed from 8:00am to 5:00pm Monday to Friday.

Self referrals

Patients can self-refer to our services by calling the Central Referral Service on 1800 600 681.

Health professional referrals

For enquiries about Child & Family Health or Complex, Aged & Chronic Care, please phone the Central Referral Service on 1800 600 681 between 8.00am and 5.00pm Monday to Friday.

There is a 24 hour voicemail message service. Messages are generally retrieved and returned within 24 hours of receipt.

Please note: If your patient is aged over 65 years (or over 50 years for Aboriginal/Torres Strait Islander patients), referrals must be made directly to My Aged Care. In My Aged Care our service provider name is Allied Health & Community Nursing & Social Support Group WSLHD

Why does CRS ask so many questions?

CRS uses Community Health Service criteria to determine the eligibility of the referral request. Demographic, family/support, legal issues, risk issues, safety issues and presenting health information is required for the client registration and clinical allocation to the appropriate Community Health service. Some information requested is required by the Ministry of Health and funding agencies.
