Primary Care and Community Health

The Primary Care & Community Health (PC&CH) Network provides a range of community based prevention, early intervention, clinical care coordination, maintenance and rehabilitative support services. The Network has four service streams:

  • Child & Family Health focuses on disease prevention, health promotion, early detection, intervention and treatment of health problems in a multidisciplinary context.
  • Complex, Aged & Chronic Care focuses on people who have multiple risk factors for chronic disease. 
  • Youth Health meets the health needs of young people aged 12 to 24 years, focusing on those who are marginalised, homeless and at-risk. 
  • Integrated Violence Prevention & Response Services provide a co-ordinated approach to violence prevention and a clinical response across the LHD for sexual assault, child protection, domestic and family violence and victims of crime.
