Referral, admission & discharge planning

Referral

To see if a referral might be suitable speak with your community nurse, GP or medical specialist, or call the unit directly.

Admission

Requests for admission can be made by your community nurse, GP or specialist doctor for assessment by the unit.

Direct admissions are possible 24 hours a day, 7 days a week.

Beds in single or double rooms are allocated according to medical need. Please note patients may be required to change rooms during admission for this reason.

Discharge planning

If indicated following an initial assessment period, the palliative care team will work with you, your carer or family in organising a plan for discharge from the unit.

Increased care options after discharge range from support and community services at home to respite or residential care in an aged care home.
