Disclosure Log

 

The disclosure log of formal access applications is a record containing details of WSLHD information already released under the Government Information (Public Access) Act 2009 (GIPA Act). A disclosure log is part of the open access information that agencies are required to make publicly available and is a requirement under Section 25 of the GIPA Act.

The disclosure log is intended to provide access to information for people who want the same or similar information as previous applicants, and would otherwise have to make formal access applications. Requests for personal information are not included in the disclosure log.  Some information that has been released under the GIPA Act may be provided in summary format only in this disclosure log.

The table below details the access applications received, the decision made on information release and how the information can be accessed.  If this information is not provided electronically below, interested parties may access copies of the information listed by contacting the Information Management Unit on 02 4734 4184.

Information from Blacktown Hospital for the period 1 July 2011 to 30 June 2012

  1. The total number of aggressive/violent incidents where security attended recorded on the Blacktown Hospital's Incident Information Management System, including incidents that occurred in the emergency department, wards, carparks and other associated Blacktown Hospital facilities.  Include incidents resulting from duress calls, pagers, urgent phone calls, recorded thefts, vandalism/malicious damage to property and assaults.
  2. Provide a summary of each of the security incidents attended to by security.
  3. The total number of OH&S incidents resolved during the period relating to 'being assaulted by another person or persons' at Blacktown Hospital, including the general occupation category of the claimant, the nature of their claim and the claims paid.
  4. The total number of risk management claims for the period by staff at the hospital, including a general occupation category of the claimant, the nature of their claim and the claims paid.
